Rapid Review Toolbar: top-bar utility in study viewer which allows for rapid review of work items

Part of ohif-viewers#21 (closed). Provide a tool for reviewing studies from a worklist in an efficient manner. Launched by selecting items from the Work Item List and selecting a "Review" button. When it launches, it displays a list of the studies selected for review, provides information about the currently selected work item (including the UID, reason for review, and status), information about the currently selected patient/study, and a control for changing the status of the work item (oak-tree/medical-imaging/imaging-development-env#66).

Details:

• Top-most section: details about patient and purpose of review ohif-viewers#75 (closed)
  • Patient details: name, DOB, MRM, sex, study description
  • Worklist item details: description of worklist item, purpose of review (attached design doe not include worklist item details)
• Navigation buttons to allow for moving between worklist items (left and right)
