Rapid Review Toolbar: patient and worklist details

Part of #21 (closed) and imaging-development-env#84 (closed). Provide a component which can display patient and worklist item details.

• Patient details: name, DOB, MRM, sex, study description
• Worklist item details: description of worklist item, purpose of review (attached design doe not include worklist item details)
