Draft: ServiceEpisodeID and ClinicalTrailSubjectID to worklist table and migration script

DO NOT MERGE. This merge request was intended to provide search/sort capability on ServiceEpisodeID and ClinicalTrialSubjectID. In the first draft of the specification, there was some discussion of including these as columns on the worklist model. **Because worklists are linked to studies however, and because the studies include references to ServiceEpisodeID and ClinicalTrialSubjectID, the implementation of the feature in Sonador 0.4 instead joins the worklist and study tables and searches via the study identifiers.
