Draft: Add ServiceEpisodeID and ClinicalTrialSubjectID to Study in apisettings to the set of fields returned by Orthanc in JSON serialization

DO NOT MERGE. Implemented to help support worklists. Because it changes the Orthanc specification (which might inject breaking changes in other places), ServiceEpisodeID and ClinicalTrialSubjectID management were merged into the ExtraMainDicomTags configuration, and the worklist representation was combined with the study representation (refer to orthanc-sonador!22 (closed)), I am marking this MR as "Dead" and closing it.
